What it addresses
The operation removes breast tissue and skin from the lower and outer parts, reshapes what remains into a smaller cone and moves the nipple to a higher position on a stalk that keeps its blood supply. The surgeon draws the pattern while you stand and decides the volume from measurements and from how the tissue feels. Tissue that is removed is not replaced, so the decision is made carefully.

How the plan is made

Complaints that come from weight often ease, because the load on the shoulders and neck falls. But back, neck and shoulder pain can have more than one cause, and posture, muscle strength and body weight are among them. The operation treats only the weight of the breast. At the examination the surgeon listens to your complaints and tells you honestly which ones may come from the breasts.
The amount follows your complaint and how much the tissue can support, not a number chosen beforehand. The surgeon measures, assesses the breast by hand and looks at the skin and the blood supply of the nipple. The aim is a breast in proportion to your body. The exact volume is not promised before the operation; the written plan gives an approximate range and the reasoning, and the final amount is settled during surgery.
Because a large amount of skin has to come away, the scar usually runs around the areola, down from the areola to the fold under the breast and along the fold itself. With a smaller removal the pattern can be shorter, but that becomes clear at the examination from measurements. Scars are red at first, fade over months and stay slightly visible. The surgeon marks the lines with you while you stand.
Breast reduction is not weight-loss surgery, so it does not replace losing weight. A stable weight is still preferred, because large changes later alter the shape and weight of the breast. If you are planning a clear loss of weight soon, say so at the examination. The surgeon discusses whether waiting would help you and decides with you.
The surgeon sets the size according to the width of your chest, your height and your shoulders, and the goal is a proportion that suits your body. In the early days the breast can look high and slightly square because the tissue is swollen. The shape softens and settles over weeks and months. The two breasts are not promised to match exactly; small differences exist in most people already.
Nothing can be promised on this. The operation can affect the milk ducts and the nerves to the nipple; some people can breastfeed afterwards, some cannot, and it is not known beforehand which it will be. If you may want to breastfeed in future, raise it at the examination. The surgeon takes it into account when choosing the technique and tells you what may be preserved, without a promise.
The stay is planned for 6–8 days, covering the operation, the dressings and the check before the flight. The surgeon decides when flying is safe. A return to work depends on whether your job involves heavy lifting or arm movements; tell the team in advance so the plan can be written around it. Avoiding heavy lifting is expected in the first weeks.
